GP Chronic Condition Management Plan
GPCCM stands for General Practitioner Chronic Condition Management, which refers to a framework and a specific type of care plan used to manage patients with one or more chronic conditions.
A GPCCM Plan is a written document that helps you identify your own role in managing your condition and achieve your treatment goals.
A GPCCM Plan can provide access to up to five Medicare-rebated allied health visits per year for patients with eligible chronic conditions.
The plan allows for referrals to and collaboration with a range of allied health professionals, such as exercise physiologists, dietitians, and psychologists.
Your GP is responsible for preparing and reviewing the plan with you and will review you every 3 months to ensure you are on track.
The fee for a GPCCMP consultation is $175.00 with a rebate from Medicare of $156.55. This will provide you with 4 consultations per year with a significantly increased rebate whilst improving your health outcomes.
